We have compiled a short equality monitoring form that is Equality Act 2010 compliant (i.e. it covers the protected characteristics). It is a sample which you can use to cut and paste, adjust, to meet the needs of your own company. Note: that the gender identity question should only be asked if those you’re asking really understand (e.g. through training) what it is you’re asking!
NB: We will shortly be posting a much more comprehensive form that will be suitable for collecting more detailed customer/service user information.
